Please bring your insurance information with you to the initial exam appointment so we can verify your benefits. Our office will assist you in processing your insurance benefits. For your convenience we accept assignment of insurance benefits. Please understand that verification of your insurance eligibility and benefit is ultimately your responsibility. We will do our best to assist you in the process but if your insurance benefit is not paid as anticipated you are responsible for the difference.
